Submission Checklist for PFC Research Grant in the Future Role of Physiotherapy in Primary Care

- Names and contact information for principal investigator and co-investigators and agency administering the funds
- Up to four (4) keywords for expertise required to review this proposal
- Summary of proposal for public release (max. 200 words)
- Rationale for project extension (up to a maximum of 15 months total) – optional
Information to be attached in individual PDFs (11-pt font, 2.5-cm margins, single line spacing)
- Cover letter (include, if applicable, previous committee concerns if a similar or related proposal has been submitted to the PFC in the past but was not funded)
- Research project proposal (max. 4 pages), which should include:
- Objectives of the research and the underlying hypothesis
- Present state of knowledge relevant to the proposal and relevant work done by the applicant
- Research design including a timeline for major activities, methodologies, analysis of results and criteria for evaluating outcomes
- Relevance of the proposal to physiotherapy
- Plans for the dissemination of results
- A list of references in acceptable scientific format (not ordinarily exceeding two pages)
- Source and amount of other funding applied for or received for this project
- Copies of the following certificates, if applicable*:
- Ethics certificate for research involving human subjects
- Subject consent forms
- Consent letters from collaborating agencies (access to data, patients)
- Certification for animal care
- Requirement for containment (biohazards)
- Completed Project Funding and Budget Information Form
- Personal data lists for all principal investigators, co-investigators, and consultants, including:
- Name, address, email
- Hours per week allocated to project
- Role on the project
- Education: university or institution and location, degrees and qualifications granted, field and year completed
- Research and professional experience (starting with present position, list training and experience in reverse chronological order: year, location, position/title)
- Publications lists (max. 2 pages) for all principal investigators, co-investigators, and consultants, including:
- List of all peer-reviewed articles published or accepted for publication during the last five years
- List of any prior publications (more than five years old) or non-peer reviewed publications directly relevant to the present application
- List of other funding awards held currently or in the last five years (including source of funding, purpose of award, amount of award, and time period of award)
