PURPOSE OF THE POSITION
Reporting to the Leader, Rehabilitation Services and as a member of the Rehabilitation Team working closely with the interdisciplinary Resident Care and Services Team, the Physiotherapist/Occupational Therapist (PT/OT) is responsible for providing clinical services to residents at LBHH and Weinberg Residence. The PT/OT assesses the individual needs of residents, and develops implements and evaluates appropriate rehabilitation plans and programs.
The services are delivered within the framework of an elderly, predominantly Jewish community with diverse ethnic and cultural experiences.
SCOPE
Located in the Oakridge area of Vancouver, B.C., Louis Brier Home and Hospital is a 215 bed accredited residential facility situated next the Weinberg Residence which is home to 40 assisted living and 20 private pay complex care residents. RESPONSIBILITIES
Develops, implements and evaluates physical / occupational therapy procedures, standards and practice.
Assesses individual resident’s needs by visiting the resident, observing mobility/behaviour patterns, reviewing the health record, and using other relevant assessment methods.
Plans and implements appropriate care plans; consults with nursing staff and resident’s physician as necessary; evaluates and documents care on an ongoing basis.
Provides day-to-day direct supervision to Rehabilitation Assistant as assigned.
Ensures safety of residents during all tasks; reports any adverse events or near-
misses to supervisor immediately.
Provides input to Interdisciplinary Care planning, through participation in conferences and Team meetings.
Reviews and recommends adaptive devices/equipment.
Provides Extended Care Dining Room support and supervision as necessary. Ensures the safety of residents during all tasks performed and reports any adverse events to supervisor immediately.
Assists Rehabilitation Team to provide staff education regarding safe work practices.
Develops, interprets and implements policies and procedures for assigned areas. Provides input to Leader, Rehabilitation Services and to relevant Committees as required.
Assists Leader, Rehabilitation Services to establish and implement CQI initiatives for Rehabilitation Services.
Reviews related equipment and supplies; recommends purchases; and ensures appropriate maintenance.
Keeps up-to-date on current geriatric rehabilitation practice through continuing education, journals, professional associations and activities. Attends meetings, conferences and workshops as a representative of LBHH when required.
Assumes other related duties/responsibilities as required.
QUALIFICATIONS:
Must be registered with either College of Physical Therapists of BC , and be eligible for membership in either Canadian Physiotherapy Association.
KNOWLEDGE, SKILLS AND ABILITIES
Strong teamwork skills.
Strong organizational skills and proven ability to deal with changing priorities and variety of responsibilities.
Strong knowledge of complex seating and swallowing assessment ( Swallowing is done in conjunction with dieticians )
Strong knowledge of falls prevention and management
Effective verbal and written communication skills.
Analytical and problem-solving approach to clinical situations.
Sensitivity to the needs and feelings of the elderly.
Sensitivity to Jewish culture, history and traditions.
Physical ability to perform required duties, and to role model appropriate body mechanics.
Hours of Work: Monday to Friday 9 am to 5 pm
