Purpose of the Position:
The Inner-City Family Health Team is a multidisciplinary team that serves homeless and previously homeless patients with complex health needs. These include untreated chronic and complex multi-system diseases, an elevated risk of acute health emergencies, and serious and persistent mental health and substance use challenges. Patients are predominantly current or former users of Seaton House Shelter; however, our mandate now includes all homeless and formerly homeless individuals of all genders from across the City of Toronto. Staff will work at our clinic on Queen Street, as well as at nearby Seaton House and potentially other community locations. Services are provided by the team using a holistic, patient-centered model of care with an emphasis on disease prevention, chronic disease management, harm reduction and health promotion.
The Physiotherapist has the knowledge, skills, and competencies to support individuals and populations across the continuum of care, including health promotion and illness prevention, self-management, treatment, and rehabilitation. The physiotherapist will assess the impact of injury, disease or disorders and its affect on the patient’s mobility and function. Physiotherapy care will encompass promoting, restoring, and prolonging physical independence by maintain and/or enhancing a patient’s functional capacity. We are seeking someone who can work with an extremely marginalized patient population, many of whom have severe mental illness, active substance use, cognitive impairment, and complex physical health issues.
Reporting to the Clinical Manager/Executive Director, the physiotherapist collaborates with the Health Team’s Family Physicians, Psychiatrists, our interdisciplinary team, and community/shelter staff to provide timely, effective, and appropriate physiotherapy in accordance with FHT policies and professional standards.
Key Responsibilities:
· Physiotherapist will have the knowledge, skills, and competencies to support individuals and populations across the continuum of care (non-pediatric in this FHT context), including health promotion and illness prevention, self-management, treatment, and rehabilitation.
· Physiotherapist will assess the impact of injury, disease or disorders of movement and function, and promote, restore, and prolong physical independence by enhancing a patient’s functional capacity.
· Physiotherapist will assess and treat individuals across the lifespan (non-pediatric in this FHT context) who have illness, injury or disability affecting the neuro-musculoskeletal, cardiopulmonary, vascular, and neurological systems; they also promote wellness and increased quality of life through education, specialized programming, and a holistic approach to care.
Experience, Credentials, Skills, and Qualifications
· Credentialing: Registration as an Independent Registered Physiotherapist in good standing with the College of Physiotherapists of Ontario.
· Experience working with marginalized patients/communities is preferred.
· Qualified Assistive Device Program Authorizer is considered an asset.
· Must demonstrate ability to conduct assessments, identify and implement care plans, document health records, and communicate and collaborate with an interdisciplinary care team.
· Bilingualism is an asset.
· Computer Skills–Basic skills related to Word processing, spreadsheets and PowerPoint; experience with, or willingness to receive training in electronic medical record software.
· Other key competencies include the ability to work in an interdisciplinary team environment; ability to prioritize, manage time effectively and be flexible in a very active work environment; compliance with practice standards from the College of Physiotherapists of Ontario
· Ability to critically appraise primary literature; and ability to use best practice health care literature databases.
· Experience working with homeless or marginalized service users preferred.
· Knowledge of and commitment to client–centred philosophy and basic principles and practice of community service delivery and chronic disease management models.
· Open minded, non-judgmental with a commitment to upholding the rights and dignity of all people and respecting their diversity.
· Excellent verbal and written communication skills.
· A sound understanding of and commitment to the Social Determinants of Health, and the ability to effectively participate in the efforts of an interdisciplinary team to contribute to comprehensive health care.
