- a brief explanation of how the results will contribute to the art and science of physiotherapy
- Brief description (max. 200 words) of your career goals and how you intend to achieve those goals

- A letter from your University’s department head confirming your acceptance into your current program of studies

- education and employment history
- graduate student supervision experience
- funding (including amounts and start and end times), and
- publications (condensed by including peer-reviewed publications for the last 5 years, together with career totals for peer-reviewed publications, non-peer reviewed publications, and abstracts)
